• I understand that all tests performed by School Health Check Ltd are entirely safe, non-invasive and are scientifically validated methods of screening and assessment in children but do not constitute a medical diagnosis without further detailed assessment by an appropriate specialist.
• I understand the limitations of screening tests done in children and that there is a low chance of the screening test failing to detect a health concern or developmental issue.
• I understand that occasionally some children will not be able to perform the test due to difficulty concentrating or lack of co-operation and in these circumstances School Health Check Ltd will not be liable for this failed testing. Parents will be informed if such circumstances arise but parents will still be liable for payment of the service provided by School Health Check Ltd.
• School Health Check Ltd reserves the right to cancel your child’s booking at short notice and rebook your child’s appointment if a specialist tester is unable to visit the school due to ill-health or attend for any other unforeseen circumstances.
• School Health Check Ltd reserves the right to cancel your child’s booking at short notice and rebook your child’s appointment if your child’s school wishes to change it’s agreed date for reasonable reasons such as staff shortage or unforeseen school closure.
• This signed e-consent form runs for 1 year from date of signing and can be used for multiple service bookings.
• I understand that any result from testing performed by School Health Check will be given to you, the parent / legal guardian and your child’s registered General Practitioner. There will be no obligation for you, as the parent / legal guardian to disclose the result to the school unless you expressly give your permission to do so.
• School Health Check Ltd will maintain the confidentiality of all records and results.

• Your child’s school and School Health Check Ltd are independent of each other and are not liable for each other’s business activities or matters regarding your child.
• If you as the parent / legal guardian are unhappy with any of the service provided by School Health Check Ltd, the company has 14 days to respond and resolve any issues directly with you.
• School Health Check Ltd has no responsibility to manage your child’s developmental issue if detected through it’s screening service but will provide appropriate clinical pathways to allow a more accurate diagnosis and further management of your child’s conditio
